We offer person-centered assistance with activities of daily living and an array of supportive health care services to qualified residents who live in publicly subsidized housing communities in Mercer County. This allows individuals to remain independent in their homes with dignity and respect. Our staff is on-site and available seven days-a-week from 7 a.m. to 11:30 p.m.
Our highly trained staff is comprised of certified and licensed team members. This unique team approach incorporates registered nursing supervision, licensed practical nursing, certified medication aides and certified home health aides to provide care and service. Our staff maintains the highest standards set by Capital Health, the New Jersey Board of Nursing and the New Jersey Department of Health and Senior Services.
Individuals who reside in publicly subsidized housing in Mercer County and meet the long-term care eligibility guidelines can benefit from our service. Qualified candidates must need assistance with at least three activities of daily living. A registered nurse will conduct a comprehensive assessment to determine eligibility and create an individualized plan of care. Insurance typically covers the cost of our services. Additionally, participants can opt to pay privately.
We collaborate with Capital Health LIFE to provide additional care to program participants and ensure continuity of care at home thus helping to avoid costly hospitalizations. We are on-site 16 hours per day, seven days a week including holidays. We often refer potential participants to one another. Currently, about half of the residents we care for are also Capital Health LIFE participants.
Including our program director, there are 14 colleagues dedicated to providing the best care to our community. Ninety-eight (98) percent of the team have been with the Assisted Living Program since its inception in March 2015. They are experts in caring for our residents, and they are part of Capital Health’s mission to improve the health and well-being of our urban and suburban communities.
