How do I get referred to Capital Health’s Center for Sleep Medicine?
Consultations with the Center’s medical staff can be arranged by calling us directly at 609-584-5150 or submitting a request through our online Contact Us form (be sure to select "Center for Sleep Medicine" under the Comment section).
Does my insurance cover a sleep study?
Sleep testing is covered by most insurances including Medicare, but you should always verify coverage with your provider ahead of time.
Testing & consultation hours
Monday through Friday: 8 a.m. - 4 p.m.
Monday through Thursday: 7:30 p.m. - 8 a.m.
Consultations are also available evenings by appointment only.
What to bring
To help expedite the registration process, please provide the following information that applies to your service.
- Identification: This will insure that the spelling of your name, social security number, and date of birth are correct. Examples of acceptable forms of ID include a valid drivers license, a valid photo ID, and/or a Social Security Card. Please note: an insurance card is not a legal form of ID.
- Insurance Information: Health insurance cards, and insurance billing address. Please note: when your insurance company requires it, please also bring pre-authorization form, referral form or both.
- Physician referral: A prescription or an insurance referral form that states the service your physician is requesting. If your insurance requires a referral and you do not obtain the referral, the cost of your services may not be covered by your insurance company.
- Self-Referral: If your insurance allows, you may refer yourself for an evaluation by our sleep physicians.
