What is a patient portal?
Capital Health’s free online patient portals give you access to your health information and valuable tools to help manage your health anytime, anywhere from your computer, tablet, or smart phone.
If you were admitted to one of our hospitals or visited one of our Emergency Departments on or after July 15, 2015, your portal will have secure access to download and e-mail your health summary to your physicians. Your summary will appear in your portal 36 hours after being discharged. Outpatient (including Emergency Department) radiology, diagnostic test results and laboratory results are available starting from July 1, 2016 and will appear in your portal 72 hours after results are sent to your ordering physician. Results from your inpatient visit will appear in your portal 72 hours after you are discharged. Additional features to help you better manage your health will become available at a later time.
It may take up to 14 days for some results to display in the portal. There are certain types of test results that we are not permitted to display in accordance with state and federal regulations.
Who can sign up for a portal?
Our portals are available to patients 18 years of age or older who see a participating Capital Health Hospital Provider. Parents or guardians of patients under the age of 18 who wish to have access to their child's medical records are asked to please contact our Health Information Management Department (Medical Records) by visiting our Contact Us form and clicking on "Medical Records" in the subject field.
How can I access my child's or other family member's health information?
You or anyone else you wish to share your information with (ie, a spouse, caregiver or other trusted loved ones). Our portal allows users the ability to share their own information with anyone else they wish. If you are trying to gain access to a child or dependent family member's health information, please contact our Health Information Management Department (Medical Records) by visiting our Contact Us form and clicking on "Medical Records" in the subject field.
Who has access to my portal?
You or anyone you grant permission to authorize your portal account on your behalf (ie, a spouse, caregiver or other trusted loved ones) through the feature “Sharing your Medical Record.”
What do I do if I lose or forget my password?
If you lose or forgot your password for the Capital Health Hospital Patient Portal and you have logged in before, click on the “Forgot your password” link on the log-in screen. Enter your e-mail address used during your registration process or the e-mail address that was given to the Capital Health representative when you registered during your visit to Capital Health. An e-mail will be sent within five minutes with a link for you to create a new password.
Is there a fee to sign up?
No, Capital Health offers portals as a free service to our patients.
Am I able to communicate with my provider through the portal?
No, this is not designed to be a communications method to e-mail Capital Health hospital physicians. If you have a question or are having a health issue, please contact you primary care provider.

Can I log-in to the Capital Health Hospital Patient Portal from my smartphone?
You can access the Capital Health Hospital Patient Portal from any computer, tablet or mobile device - including smartphones - with internet connection. The portal has user-friendly navigation, however, it may be easier to to view information on your computer or tablet rather than your smartphone, because the portal is not yet optimized for mobile viewing.
