My personal information is not correct. What do I do to update it?
If your personal information is not correct from your last visit to one of our hospitals, please contact our Health Information Management Department (Medical Records) by visiting our Contact Us form and clicking on "Medical Records" in the subject field. The information in the Capital Health Hospital Patient Portal should reflect your name, address, phone number, and insurance information that were current at the time you were at Capital Health for your test, Emergency Department visit, or inpatient stay. Any changes after your last visit in your name, address, phone number, insurance information will be updated at your next visit to Capital Health.
I recently had tests done. How do I see the test results?
It may take up to 14 days for some results to display in the Capital Health Hospital Patient Portal. There are certain types of test results that we are not permitted to display in accordance with state and federal regulations.
What if I have a question about the health information I am seeing?
Please contact your primary care provider for questions about your health information.
