Capital Health LIFE is a Program of All-Inclusive Care for the Elderly (PACE) service. PACE is a Medicare and Medicaid program that helps older people live more independently by meeting their health care needs in the community instead of going to a nursing home or other care facility. PACE health care professionals work closely with participants and their families to provide the coordinated care they need to maintain and improve their health.
Capital Health LIFE's holistic approach goes beyond medical services. As an alternative to nursing home care, we offer several significant advantages:

Capital Health primary care providers, nurse practitioners, registered nurses, therapists, social workers, dietitians, home health aides, and others work together to create personalized care plans tailored to each participant’s health needs.
- Care that helps them live safely at home for as long as possible. We support the independence seniors want and the dignity they deserve. We do this by keeping them connected to the place where they are most comfortable—in the community and the place they call home.

Our team coordinates all services needed to provide personalized care plans for seniors and eliminate the stress for caregivers that comes with managing appointments, medications and more. This provides peace of mind to both participants and their loved ones.
- Support services that make a difference. We understand the impact mental health has on physical wellness, and our social support services prioritize seniors' quality of life. Our Social Workers advocate and connect participants to resources and more to ensure participant and caregiver needs are met and addressed.
