Services Provided

Referral-Based Program

A referral from your physician is required to participate in our Diabetes Self-Management Education and Training Program, refresher classes, and individual appointments.

Your doctor may refer you to our program during your appointment with them. If not, please ask your medical provider for a referral to the Capital Health Diabetes Education Center so we can contact you to set up an appointment. Alternatively, you can contact us directly so we can reach out to your medical provider to get a referral for you.

Most insurance plans are accepted, including Medicare. For steps on how to ask your insurance provider about coverage for diabetes and nutrition education, call 609-537-7081.

Diabetes Self-Management Education and Training Program

This is a referral-based, comprehensive outpatient education program that empowers individuals to take control in the successful management of their diabetes. In addition to following your physician’s guidance, managing diabetes requires the skills to make healthy decisions every day. Learn from a certified diabetes care education specialist, nurses, dietitians, and other health care providers to gain the knowledge and skills to manage your diabetes with confidence.

Individual Initial Assessment Appointment

Before attending the four-week group class series, you’ll meet with a diabetes educator to complete an initial assessment appointment. These comprehensive 60-minute individual sessions include an evaluation of self-care behaviors such as problem-solving, managing hyperglycemia/hypoglycemia, and the ability to self-manage blood glucose and insulin dosing on a day-to-day basis. Offered in either in-person or virtual options, you’ll discuss your individual challenges and set goals for managing your diabetes.

Group Classes

This four-week series (with 90-minute classes) covers various topics to help manage your diabetes:

Class 1: Diabetes management, pathophysiology, chronic complications, medications (oral and GLP-1).
Class 2: Healthy eating (part 1), lifestyle and healthy coping.
Class 3: Monitoring, acute complications, medication (insulin), distress and support.
Class 4: Being active, healthy eating (part 2).

Daytime and evening classes are available. Daytime classes are held Wednesdays from 9:30 – 11 a.m. at Capital Health – Hamilton. Evening classes are held Thursdays from 5:30 – 7 p.m. at Capital Health Medical Center – Hopewell.

Individual Follow-Up Appointments

  • After completing group classes, follow up with our educators to review progress on goals and discuss areas that need additional attention and personalization.
  • Meet with a diabetes educator to learn how you and your family can learn to live well with any type of diabetes.
  • A registered dietitian (RD) will work with you to create an individualized meal plan to help manage diabetes and a variety of nutrition concerns.
Refresher Classes

These are referral-based diabetes education classes for those who have completed the Diabetes Self-Management Education and Training Program. Offered periodically, these classes explore up and coming diabetes self-management topics.

Free Programs

Diabetes Support Groups

Held virtually on the third Wednesday of each month from 5:30 - 6:30 p.m.

Managing diabetes can be a lot of work, so it’s important to get support from other people who are living with it. Led by our caring, supportive guides, you will connect with others living with diabetes to share experiences and insights on managing diabetes and overall health. Our support groups discuss all topics related to daily self-management, including daily care adherence, finances, and nutrition. Additional benefits include:

  • Giving and getting support
  • Offering and receiving practical insights about dealing with diabetes
  • Sharing challenges, stresses, and successes
  • Minimizing the isolation you may feel living with and caring for your diabetes
