To ensure all credentials are received, please send them in one packet by the February 28 or March 31 deadline. A completed application includes the following:
- Applicant Submission Form
- Application for Admission
- $50.00 nonrefundable application processing fee, payable by cashier’s check, certified check, or money order only, made payable to Capital Health School of Nursing.
- One (1) official high school transcript.
- Two (2) official transcripts from ALL other educational institutions you have attended since high school. Official transcripts must be in separate sealed envelope with an official signature or stamp across the seal.
- Three (3) professional references (teacher/instructor, employer/supervisor, community service representative/clergy).
- Transcript of a high school level Chemistry course with a minimum "C" grade that is less than 10 years old at the time of admission (A college level Basic Chemistry course or a credit-granting score on the CLEP exam is also acceptable).
- Official Entrance Exam results (TEAS all applicants; TOEFL International applicants). If proctored by Capital Health you will not need to include the results.
